History:A32-year-oldmanpresentswithchronicrightkneepainfollowingafallsixmonthsprior.
病史：32岁男性，6个月前摔倒后慢性右膝疼痛。
Radiographsoftherightkneeareshownbelow.
右膝平片如下所示。
MRIoftherightkneewasattainedforfurtherevaluationofthepatient’ssymptoms.
进一步行右膝MR扫描。
Findings
Radiographs:Frontalandlateralradiographsoftherightkneedemonstratenoevidenceoffractureordislocation.Thereisanelongatedscleroticlesionextendingalongtheanteriorproximaltibialmetadiaphysis.Nocorticaldestructionisseen.
MRI:Sagittalfat-saturatedprotondensityimageoftherightkneedemonstratesahypointenselesionalongtheanteriortibialcortex,correspondingtotheaboveradiographicfindings.Thereisnosurroundingmarrowedema.ThelesionisalsohypointenseonthecoronalT1-weightedimage.
平片：右膝正侧位片示未见明显骨折或脱位，可见一纵行硬化性病变沿胫骨近段干骺端前部走行。未见明显骨皮质破坏。
MRI：右膝矢状位脂肪抑制质子密度成像示低信号病变沿胫骨前缘皮质分布，和平片图像相对应，周围未见骨髓水肿，冠状位T1WI示病变也呈低信号。

Thereisapredilectionforthelongbonesofthelowerextremities.
Usuallyaffectsasingleextremity(canhavesingleormultiplefoci).
Etiopathogenesisispostulatedtobedevelopmental,relatedtodysfunctionalmesodermaldevelopmentinalimbbud,orasaresultofaninsulttospinalaccessorynerves.
Itisasporadic,nonhereditarycondition,butithasbeenassociatedwithmutationsintheLEMD3gene.
Naturalhistory:
Rateofhyperostosisisfasterinpediatricthanadultpatients.
Ittendstoprogressslowly,witheventualstabilizationduringadulthood.
病理生理：
烛泪样骨质增生症是一种由于膜内成骨的缺陷而导致的不规则的条纹状骨肥厚。
好发于下肢长骨；通常累及单侧（可单发或多发）
发病机理认为可能与胚芽时期中胚层发育不良有关，或者与脊副神经的损害有关。
散发，非遗传性，但可能与LEMD3基因突变有关。
自然病程：小儿骨肥厚的比例高于成人；进展缓慢，在成人后最终稳定化。
Epidemiology
Typicallydiagnosedinadolescenceorearlyadulthood.
Thereisnogenderpredilection.
流行病学
多在青春期或成人早期诊断此病；
无性别优势。
